Diabetes Prevention Program In-Person
Southwest District Health is offering a Diabetes Prevention Program.
You may be eligible to participate in the program if you have:
- been diagnosed with prediabetes
- ever been diagnosed with gestational diabetes
- received a high risk result on a prediabetes risk test
